WHO IS ELIGIBLE?
Patient age:
- Birth to 21 yrs of age
- Infants with a congenital or acquired life-threatening or life-limiting medical condition
- Expectant parents who anticipate delivering a baby with a problem that is not compatible with life.
Limited to New Jersey residents. Target counties include:
- Essex
- Hudson
- Union
- Morris
Qualifying Medical criteria:
- Children with progressive life-threatening illnesses or life-limiting medical conditions whose quality of life is negatively impacted by pain and/or symptoms caused either by trauma. illness or consequent treatment;
- Children diagnosed with progressive or life-limiting medical conditions where there is the expectation that COLCC interventions and programs will positively impact their quality of life.
- Children with progressive life-threatening illnesses or life-limiting medical conditions whose family may benefit from assistance in resolving complicated family issues related to the child’s deteriorating medical status;
- Fragile infants born with or affected by life-limiting medical conditions and expected to live a significantly shortened and medically complicated life.
The above criteria serve as guidelines only as decisions are made on a case-by-case basis.
